Registration for 6-12 grade athletes for the 2024-2025 school year will open on July 15th, 2024

WIAA Physical Examinations
All student-athletes participating in interscholastic athletics must have a completed WIAA Sports Physical form on file at the D.C. Everest Athletics Office prior to practice or participation. Each physical form must be signed and dated by a licensed Physician (MD or DO), PA or APNP.
Date of actual medical physical:
Before 03/31/2023: the student-athlete will need a new physical. Please complete a physical form signed and dated by the medical professional and the parents/guardians. Submit via this registration site or bring a copy to the Athletics Office.
From 04/01/2023 - 03/31/2024: the student-athlete will need to submit an Alternate Year Form. Submit via this registration site or bring a copy to the Athletics Office.
From 04/01/2024 - Present: Please complete a physical form signed and dated by the medical professional and the parents/guardians. Submit via this registration site or bring a copy to the Athletics Office.
Insurance Policy
All student-athletes are encouraged to be covered by private insurance before participating in a sport. Student accident insurance is offered for purchase by the D.C. Everest Area School District. Student accident insurance plan forms may be obtained through the RSchool online registration or from the office at each school. The D.C. Everest Area School District does not carry insurance coverage for individual students.
Athletic Fees & Waivers
Parents can make payment for athletic fees through the Infinite Campus Student Portal. Fees will not be posted to the student portal until the beginning of August. Please check payment status HERE.
Fee waiver forms cannot be submitted online. A printable version of the form is available under the ``Other Forms`` tab. Completed fee waiver forms must be returned to the athletics office at your child`s school.
